A case manager may be a person who gathers data so evaluates, assesses, advocates and plans resources for his/her client. The client could also be one person or an entire family. The case manager provides the client with quality and efficient services.

Although case managers add a lot of environments, some common parts include:

Assessment, that is the method of identification of the condition/needs, skills and preferences of the individual, which results in the event of an idea of care.
