The introduction of heart valve replacement surgery in the early 1960s significantly improved outcomes in patients with valvular heart disease. Approximately 90,000 valve replacements are currently being implanted in the United States and 280,000 worldwide each year; about half are mechanical heart valves and half are bioprosthetic valve replacement. Despite important improvements in valve prosthesis design and surgical procedures over the past few decades, valve replacement does not provide a definitive cure for the patient.  Aortic valve stenosis In contrast, natural valvular disease is exchanged for “prosthetic valve disease” and the outcome of patients undergoing valve replacement is affected by the hemodynamics, durability and thrombogenicity of the valve prosthesis. However, through the selection of the optimal prosthesis for each individual patient and careful medical care and follow-up care after implantation, many of the complications associated with the prosthesis can be prevented or their effects minimized.  aortic valve replacement The aim of this article is to provide an overview of the current state of knowledge and future prospects with respect to prosthesis choice and optimal clinical management after valve implantation.
