The Gamma Knife employs a technique known as stereotactic radiosurgery, which involves the use of several beams of radiation converging in three dimensions to precisely focus on a small volume, such as a tumour, allowing strong doses of radiation to be administered safely to that volume. Presently available models employ advanced robotic technology to move the patient in submillimeter increments during treatment in order to successfully focus radiation on all portions of the target.


The majority of treatments are delivered in a single session. Under local anaesthesia, a customised rigid head frame with a three-dimensional coordinate system is screwed to the patient's skull. The results of imaging investigations such as magnetic resonance imaging (MRI), computed tomography (CT), or angiography are then submitted to the Gamma Knife planning computer system. Physicians (radiation oncologists and neurosurgeons) and medical physicists work together to define targets and normal anatomical features.


To calculate Gamma Knife treatment settings, they employ a planning computer software to identify the exact spatial relationship between the target, normal structures, and the head frame. Targets are frequently best treated throughout the therapy session with a series of successive highly targeted treatments, colloquially known as "shots." Many fine-tuning modifications of treatment parameters are frequently considered by physicians and physicists until an optimal plan and dose are identified.
