Pulmonary Drugs: An Overview of Available Medications

Bronchodilators

Bronchodilators are one of the most commonly prescribed classes of pulmonary drugs. They work by relaxing and opening the airways of the lungs to make breathing easier. There are two main types of bronchodilators - short-acting and long-acting. Short-acting bronchodilators like albuterol provide fast relief from bronchospasm or a constricted airway but only last for a few hours. They are usually administered through an inhaler as needed. Long-acting bronchodilators like salmeterol provide relief for a longer duration, sometimes up to 12 hours. They help control symptoms over an extended period and are generally taken twice daily.

Besides relaxing airway smooth muscles, bronchodilators also alleviate coughing, wheezing and shortness of breath associated with conditions like asthma, COPD and emphysema. Common bronchodilating drugs include beta agonists like albuterol, formoterol, salbutamol and anticholinergics like ipratropium bromide, tiotropium. Their use has revolutionized the management of obstructive pulmonary diseases.

Corticosteroids

Corticosteroids are another important class of drugs used to treat various lung disorders through Pulmonary Drugs. They work by reducing inflammation in the airways and lungs. Inhaled corticosteroids (ICS) are commonly prescribed for conditions such as asthma where airway inflammation plays a major role. ICS medications include beclomethasone, budesonide, fluticasone and mometasone. They have minimal systemic side effects as they are delivered directly to the lungs through inhalers.

ICS are very effective in gaining control over asthma symptoms and reducing exacerbations. They are also used together with long-acting bronchodilators in maintenance therapy. Oral corticosteroids like prednisone are sometimes used short-term during asthma attacks to rapidly reduce airway inflammation and relieve symptoms. Long-term use of oral steroids is not recommended due to risk of serious side effects. Overall, inhaled corticosteroids form an indispensable class of pulmonary drugs.

Anti-infective Agents

Lung infections greatly compromise our respiratory health. Thankfully, modern medicine has a variety of anti-infective drugs that effectively treat most pulmonary infections. Antibiotics are commonly prescribed to treat bacterial lung infections like pneumonia, bronchitis, tuberculosis etc. Common antibacterial drugs include amoxicillin, azithromycin, clarithromycin and levofloxacin. Anti-viral drugs are used for viral lung infections caused due to influenza, respiratory syncytial virus (RSV) and others. Oseltamivir, zanamivir, ribavirin etc. are some patented anti-viral drugs available in the market.

Anti-fungal medications play a key role in treating fungal infections of the respiratory tract. Pneumocystis pneumonia, a life-threatening fungal infection commonly seen in AIDS patients, responds well to drugs like trimethoprim-sulfamethoxazole and pentamidine. Multi-drug resistant tuberculosis (MDR-TB) is effectively managed with a combination of second line injectable and oral antibiotics. With the availability of potent anti-infectives, most pulmonary infections can now be successfully treated.

Mucolytics

Mucolytics are drugs that work by reducing bronchial secretions or thickening of mucus in the lungs. Copious amounts of viscous mucus cause significant breathing problems in lung conditions like chronic bronchitis and cystic fibrosis. Commonly used mucolytic agents include acetylcysteine, bromhexine and iodinated glycerol. They work by breaking disulfide bonds between mucoproteins, thus thinning out mucus secretions and making them easier to cough up.

Reduced viscosity of mucus facilitates its mobilization and clearance from the lungs. This provides effective relief from coughing, wheezing and sputum production associated with excess bronchial secretions. Mucolytic therapy, along with chest physiotherapy, forms an integral part of managing chronic lung diseases characterized by mucus hypersecretion. By easing the work of breathing, mucolytics go a long way in improving quality of life.

Targeted Therapies

In recent years, drug research has led to the development of targeted therapies for lung conditions. These highly innovative drugs work by influencing specific molecular pathways involved in disease pathogenesis. For instance, tyrosine kinase inhibitors like gefitinib and erlotinib target mutations in epidermal growth factor receptor (EGFR) seen in some lung cancers. They have transformed the prognosis of non-small cell lung cancer patients with these mutations.

Similarly, angiogenesis inhibitors like bevacizumab work by blocking new blood vessel formation in tumors, restricting their growth and spread. Cystic fibrosis (CF) patients now have the option of precision medicines like ivacaftor that target the defective CFTR protein and restore its chloride channel function. Such targeted therapies have minimal side effect profiles while offering hope for effective treatment where other options have failed. They exemplify the promise of precision or personalised medicine approach.

Development in drug research continues to expand our therapeutic armamentarium against respiratory diseases.
