Dangers of Darvocet, FDA Warnings, and Treatment

Propoxyphene is an opioid medicine that was licensed by the Food and Drug Administration (FDA) for the first time in 1957. It has been used to treat mild to moderate pain as a particular ingredient (e.g., Darvon) or in addition with acetaminophen (e.g., Darvocet) in the United States since 1976 as a Schedule IV Controlled Substances. Since 1978, FDA has received two citizen petitions requesting the removal of propoxyphene from the market or rescheduling the drug from Schedule IV to Schedule II (moving from a category of drugs with a low chance of dependence to a category reserved for drugs with more potent effects).


The FDA held a meeting of its advisory group in January 2009 to discuss propoxyphene's efficacy and safety. The committee voted 14 to 12 against propoxyphene's continuing marketing but stated that more information concerning the drug's cardiac effects would be useful in assessing the risks and benefits.


The FDA ruled in July 2009 to allow propoxyphene's continuing marketing if a new boxed warning was added to the medicine label, warning consumers and health care providers of the danger of a lethal overdose. Similarly, Xanodyne conducted a trial in healthy volunteers to establish the best dose for the definitive cardiac study. Even at recommended levels, propoxyphene caused significant alterations in heart electrical activity, including a longer PR interval, enlarged QRS complex, and prolonged QT interval, according to the findings.


In light of these new scientific results, the FDA judged that the drug's post-marketing safety signals had increased in relevance, and the overall risk-benefit balance can no longer be regarded as favorable. Propoxyphene products should be removed from the market in the United States, according to the FDA.
