Father's name- In this section of the form, the applicant must include their father's first, last, and surname. This applies only to individual applicants, and married women should fill out this section of the form as well.

Address- In this section of the form, the applicant must enter their home and business addresses. The applicant must be cautious when entering the address and provide accurate information.

Communication address- The applicant must select the address at which they wish to be contacted.

Telephone number and email address- For communication purposes, the applicant must include their country code, state code, and telephone/mobile number.

Status of applicant- In this section, the applicant must specify whether they are an individual, a HUF member, a company, a partnership firm, etc.

Registration number- The registration number is the number assigned to a company, firm, or LLP.

Citizenship- In this section of the form, the applicant must specify the country of citizenship and the ISD code of that country.

Representative of the applicant in India- In this section, the applicant should include information about the representative or agent, such as their address and full name.

Documents submitted- In this section of the form, the applicant must list all of the documents that they have submitted along with the form.

KYC details- According to SEBI regulations, this section of the form must be filled out by a Qualified Foreign Investor. This will include information such as marital status, occupation, citizenship status, and more.
